If at any time you have questions about your recredentialing, please contact the Credentials Verification Office at Sheet If you have any difficulties accessing the portal, please refer to our Initial Credentialing Online Portal Application Tip Sheet or contact the CVO. Once the application request is processed (in approximately 5-7 days), a welcome email will be sent to the provider and the delegate with a link to our online portal application. Providers are required to sign into the Provider Portal to review all information, complete the disclosure questions and the delineation of privileges forms (if applicable), sign and submit the application. Delegates may NOT complete disclosure questions or the delineation of privileges form (if applicable), nor sign and submit an application on a provider's behalf. This delegate may complete the demographic, malpractice, licensure, schooling and other informational portions of an initial application or recredentialing application.
